Accommodative Esotropia with High AC/A Ratio in Toddlers: Executive Flat-Top Bifocal Placement at Mid-Pupil

The Accommodative Convergence to Accommodation (AC/A) ratio measures prism diopters ($\Delta$) of convergence induced per diopter of accommodation:

In toddlers with a High AC/A ratio ($>8:1$), focusing on a picture book or toy $33\ \text{cm}$ away requires $+3.00\ \text{D}$ of accommodation, driving an astronomical $\mathbf{24\ \Delta}$ to $\mathbf{30\ \Delta}$ of inward convergence! The eyes violently cross, destroying stereoscopic 3D depth perception.

Treatment requires two immediate optical actions:

1. Full Cycloplegic Distance Correction: Under 1% cyclopentolate or atropine, every fraction of hyperopic power must be prescribed (no cutting of plus!).
2. High Near Addition ($+2.50\text{ to }+3.00\ \text{D}$): Relaxes near accommodation to zero, mechanically extinguishing excessive near convergence and straightening the eyes.

In adults, bifocal lines sit at the lower eyelid. If fitted this way on a child, the child simply dips their head and looks over the bifocal line to look at toys, remaining completely crossed! Therefore, the segment line of an Executive or Flat-Top 35 bifocal must be positioned to bisect the center of the pupil (mid-pupil) in primary gaze. Every downward eye saccade is forced through the relaxation segment.
