Defining Low Vision: When Standard Refraction Reaches Its Limit
The World Health Organization defines Low Vision as visual acuity worse than 20/70 (6/18) in the better-seeing eye that cannot be corrected by standard eyeglasses, contact lenses, medications, or surgery. The patient is not completely blind, but normal activities like reading books, recognizing grandchildren's faces, or signing checks become impossible.
The fundamental principle of Low Vision optics is Retinal Image Magnification: if a damaged central fovea cannot resolve a 10-point font, optical magnification enlarges the image by 2x, 4x, or 8x, spreading the visual information across healthy, undamaged parafoveal retinal cells.
The 4 Categories of Optical Low Vision Systems
- High-Addition Prismatic Microscopes (+4.00D to +12.00D): High plus lenses that allow the patient to hold reading material 8 to 12 centimeters from their face. To prevent severe eye strain from excessive convergence, base-in prisms (Prentice rule compensation) are ground into both lenses to allow comfortable fusion.
- Galilean Telescopic Spectacles: Utilize a positive objective lens and a negative ocular lens. Galilean systems provide 1.7x to 2.5x magnification with a bright, upright image and lightweight footprint, ideal for watching television.
- Keplerian (Prismatic) Telescopes: Utilize two positive lenses with an internal roof prism to invert the image. They provide powerful magnification (3x to 6x) with a wide, edge-to-edge sharp field of view, but are longer and heavier.
- Bioptic Driving Telescopes: A miniature telescope mounted into the upper carrier lens of an ordinary eyeglass frame. The patient drives looking through the carrier lens 95% of the time, occasionally dipping their chin to glance through the telescope for 1 second to read distant street signs or traffic lights.
