Historically, prescribing doctors left PD measurement to the dispensing optician because the physical measurement depends on the patient's chosen frame width, vertex distance, and pantoscopic tilt. In the digital age, however, ordering precision glasses online requires knowing your exact PD to ensure the lens optical center (OC) aligns millimeter-perfect with your visual axis.
Prentice's Rule: The Mathematical Cause of Eyeglass Headaches
If your optical centers do not align with your pupils, the lens behaves as an unintended prism according to Prentice's Rule:

For example, if you wear a -5.00D prescription and the optical center is displaced by just 3mm (0.3cm) due to an inaccurate PD measurement:

Monocular PD vs. Binocular PD: The Asymmetry Rule
Most human faces are slightly asymmetrical. A single Binocular PD (e.g. 64mm) assumes each eye is 32mm from the nasal bridge. In reality, your right eye might be 31mm and your left eye 33mm (Monocular PD: 31/33mm).
For prescriptions above ±3.00D and for all progressive corridor lenses, insisting on Monocular PD is mandatory to avoid corridor swim and blurred reading zones.
